I have this sentence that I find confusing. I have tried rewording. This is the original. Wonder can you suggest a rewording:

'The study was in the form of life style questionnaire linked answer from a list of food questions to specific food frequency items proceeded by behavioural questions related to patients lifestyle. '

Wowzer. That is pretty confusing. It needs a complete rework. How about:

'The study derives its finding from a questionnaire based on patients' consumption levels of specific food items and life style.'
