Lists - using "or" or "and"

Which is most correct:

"There is no evidence of high grade dysplasia or malignancy."

vs

"There is not evidence of high grade dysplasia and malignancy."

The first sentence is definitely the right choice, as the second one merely precludes the possibility of both but not of either one alone. In other words, although there may not be BOTH high grade dysplasia or malignancy, there may, though, be EITHER.

Grammatically, both are fine. But, they mean different things. (You could put a hyphen in high-grade.)
