Grammar question

Hi,

Is this sentence grammatically correct?

Any such appeal must be made in writing, addressed to the Warden (Mr John Smith), briefly setting out the grounds upon which the appeal is based, and be received within ten days of the date of this letter.

I think it should be:

Any such appeal must be made in writing, be addressed to the Warden (Mr Robert Wilson), briefly set out the grounds upon which the appeal is based, and be received within ten days of the date of this letter.

Thanks,

Tom

Re: Grammar question

I think they're both okay.

Any such appeal must:

(1) be made in writing, addressed to the Warden (Mr John Smith), briefly setting out the grounds upon which the appeal is based, and
(2) be received within ten days of the date of this letter.

Any such appeal must
(1) be made in writing,
(2) be addressed to the Warden (Mr Robert Wilson),
(3) briefly set out the grounds upon which the appeal is based, and
(4) be received within ten days of the date of this letter.

I would chop it up for clarity:

Any appeal must made within ten days of the date of this letter and submitted in writing to the Warden (Mr Robert Wilson). It should briefly state the grounds for the appeal.

There's a section on business writing here. (This site is under construction still, but there's enough there to be useful.)
