On the subject of healthcare, I don't know ( who or whom) to believe.

The short answer is that both are okay.

In your example, the wh- word is object of "believe", so strictly speaking the objective pronoun "whom" is correct. But it's rather formal, and subjective "who" is preferred in conversational spoken English by most people.
