Which one is correct
1.
"Please note, this is a one-time exception, if additional services are obtained there it will be subject to the deductible."
2.

Please note, this is a one-time exception. If additional services are obtained there it / they will be subject to the deductible.


The pronoun refers to the plural noun "services", so it must be the plural "they".

Incidentally, you need some kind of terminal point or a semi-colon or dash after the word "exception". I've inserted a full stop.
