the "by whom" bit is correct. However, I think you may need to re-word your sentence.

Without knowing the context, it would be difficult to advise the second half of the sentence, but I think you may need to make the "performed by whom" bit a little clearer.
...he wants to know the exact time and date and performed by whom.
...he wants to know the performer and the exact time and date.
