2008 Financial Reporting Packages


This notice serves as a reminder that all holders of taxicab medallion licenses are required by section 9-112-145 of the Municipal Code of Chicago to submit financial information.

Please be advised that the deadline for submissions has been extended to May 29, 2009.

Information must be submitted on the Department-issued form, “2008 Financial Reporting Package.” This form is available:

• Online at www.cityofchicago.org/bacp

• At the Public Vehicle Division Offices, 2350 W. Ogden


Any inaccuracies or late submissions will put the medallion owner in violation of section 9-112-145(g) of the Municipal Code of Chicago. This section states the failure to submit this required information shall result in a fine of not less than $200 and not more than $750, as well as suspension or revocation of your taxicab medallion license.
